NeuroResource Facilitator for Counties in Central Pennsylvania

The Brain Injury Association of Pennsylvania, in collaboration with the PA Department of Health, is working to connect individuals with brain injuries to care across the Commonwealth of Pennsylvania. This five year project aims to connect individuals with treatment, resources, and benefits, as well as to build natural supports in the community. BIAPA is currently hiring NeuroResource Facilitators (NRFs) to take on this important work. 

NRFs will provide short term care management with individuals referred to the project to identify goals, link them with applicable local resources and ensure access to identified resources. NRFs will make at least one home visit with each client, though most services will be provided via telehealth platform or by phone. NRFs will document their work electronically and collect relevant data to track clients’ progress. 

NRFs will be part-time independent contractors for this multi-year, grant-funded project. Each NRF will be responsible for one of 5 regions of the Commonwealth. Requirements include: Bachelor’s degree, valid driver’s license, and 5 years of professional experience working with individuals with brain injury as a case manager, rehabilitation health professional or related healthcare field. Applicants must be Certified Brain Injury Specialists (CBIS). Specific job-related training will be provided. 

BIAPA is currently seeking an NRF to provide this service in Philadelphia. The NRF does not need to live in Philadelphia but must be able to meet with clients in Philadelphia in their community at least once during the period of service.
